Invitation
As the Convenor of the Healthcare @ Home Conference hosted by the Australian Council of Community Nursing Services Inc. I wish to invite you to participate in this exciting conference.
This conference only comes around every two years and this year we have decided to take this unprecedented step to incorporate a multi-disciplinary focus to the conference. We have invited speakers who we believe can provide background and vision for how collaboratively we can continuously improve to provide safe and quality health services in the community for the benefit of all Australians.
The concurrent sessions reflect the diversity we seek in the program. I encourage you and your colleagues to take this opportunity to gain recognition for the valuable contribution you make to so many people living in the community.
The culmination of the conference with be a hypothetical facilitated by Dr Norman Swan. The question posed is “In the future, most healthcare will be provided in the community”. The hypothetical session is addressing this topic & will canvass the impact of increasing the proportion of healthcare provided in the community settings; what is required to achieve this goal in a quality, safe and sustainable manner; and what challenges does the healthcare profession face into the future. An expected outcome of the hypothetical would be that conference participants would develop a clearer vision and direction for the future of the healthcare profession.

Scientific Program
The Magic, The Mystery, The Moments – the future is in your hands" has been chosen as the theme for our Healthcare@Home Conference in 2005. This will be an important opportunity to reflect on our achievements in healthcare@home to date and to ponder the future of an increasing proportion of healthcare being provided in the home environment. Our diverse range of invited international, national and local speakers will stimulate your thinking culminating in a hypothetical led by Dr Norman Swan. Concurrent papers will cover areas such as Chronic Disease Management, Quality and Safety, Post Acute Care, Dementia, Child and Family Health, General Practice, and the role of carers. The program will be held over two days and an exciting social program taking advantage of all that Sydney has to offer will also allow the opportunity to catch up with colleagues and meet new friends.
This program has been endorsed by Royal College of Nursing, Australia, according to approved criteria. 12 continuing nursing education (CNE) points have been allocated to the activity, which may be used by nurses as part of their enrolment in the Royal College of Nursing, Australia Life Long Learning Program (3LP).
