
Registration
Those who wish to attend the Workshop should complete the online Registration Form or fill in the Registration Form in the brochure and return it to the Workshop Managers. Each registrant must fill in a separate form. A clean photocopy of the form will be accepted, or additional forms can be obtained from the Workshop Managers.
Please note that all persons intending to attend the Workshop must register, including invited speakers, authors and chairpersons.

Payment Of Fees
NOTE: Registrations will not be processed or confirmed until payment in full is received.
Within Australia
Payment may be made by personal cheque or credit card. Mastercard and Visa are the only credit cards accepted at the Workshop. Please note all transactions by credit card will appear on your statement as payment to Conference by THPL. Cheques should be made payable to the MM10 Workshop Managers.
International
Payments from overseas must be made by credit card or bank cheque in Australian Dollars only, drawn on an Australian Bank. Your name and full address should be typed or printed clearly on the back of the cheque. You may not pay your fees by direct transfer.
Acknowledgements
Your registration and payment will be acknowledged in writing with confirmation of your requirements according to your registration form. Your registration will not be processed or confirmed if payment does not accompany your form.
You will not receive any further publications before the Workshop. Your letter of acknowledgment will include any further information required. At the Workshop you will receive the final program, Haematology Journal (Workshop Proceedings) and a list of delegates.
Entrance To Workshop
Each member of the Workshop and Accompanying Persons will receive a name badge on registration. The badge will be your official pass and must be worn to obtain entry to all sessions and to social functions.
Letter Of Invitation
On request, the Organising Committee will send a letter of invitation to participate in the Workshop. Such an invitation is extended specifically to assist participants to obtain travel funds or appropriate visas and does not imply any commitment on the part of the Organiser to provide financial support. Please complete section L of the registration form to request a letter. The letter will be forwarded to you with confirmation of your registration.

The Registration Desk will be located on Level 2, Business Centre South and will be operating as follows:

The Workshop Managers will be on-site for enquiries at other times. A Tours & Hospitality Services Desk will also operate during Workshop hours.
Insurance
Registration fees do not include insurance of any kind. It is strongly recommended that at the time you register for the Workshop and book your travel you take out an insurance policy of your choice. The policy should include loss of fees/deposit through cancellation of your participation in the Workshop, or through cancellation of the Workshop, loss of international/domestic air fares through cancellation for any reason, loss of tour monies through cancellation for any reason including airline or related services strikes within Australia, failure to utilise tours or pre booked arrangements due to airline delay, Force Majeure or any other reason, medical expenses (including sickness and accident cover), loss or damage to personal property, additional expenses and repatriation should travel arrangements have to be altered. The Workshop managers cannot take any responsibility for any participant failing to arrange their own insurance. This insurance is to be purchased in your country of origin.
Private Touring/Hospitality Arrangements
Should you have any specialised group requirements for private touring, transportation or hospitality services other than those organised for the official program, please contact the Workshop Managers who would be pleased to assist with arrangements.
Private Functions
It is requested that you advise the Workshop Managers of any private /corporate functions that might be planned outside of the official program. The Workshop Managers would be pleased to assist with arrangements for any of these events.
Cancellation And Refund Policy
Cancellations must be notified in writing to the Workshop Managers.
Registration Fees
Before 14 January 2005 will incur a A$100.00 cancellation fee.
Before 25 February 2005 will receive a 50% refund of fees paid.
After 25 February 2005 there will be no refund.
