*Nurses and students are required to email/fax a copy of their Identification Card to the Congress Managers to be eligible for this rate.

To be entitled for the Early Registration fee you must have registered and paid by 3 August 2009.

Congress Delegate Entitlements

The Congress registration fee (categories A-F) entitles all delegates to the following:

Payment of Fees

NOTE: Registrations will not be processed or confirmed until payment in full is received.

Within Australia
Payment may be made by cheque or credit card. Please note all transactions by credit card will appear on your statement as payment to ‘Conference by THPL’. Cheques should be made payable to the 13th Australasian Menopause Society Congress.

International
Payments from overseas must be made by credit card or bank cheque in Australian dollars only, drawn on an Australian bank. Your name and full address should be typed or printed clearly on the back of the cheque. We are unable to accept direct bank transfers for registration fees.

Acknowledgements

Your registration and payment will be acknowledged via email with confirmation of your requirements according to your registration form. Your registration form will not be process or confirmed if payment does not accompany your form. You will not receive any further publications before the Congress. At the Congress you will receive the final program and a list of delegates.

Cancellations must be notified in writing to the Congress Managers.

Registration Desk

The Registration Desk will be located on the Ground Floor of the National Convention Centre Canberra and will be operating as follows:

Friday 18 September 2009               8.00am – 7.00pm
Saturday 19 September 2009          7.00am – 3.30pm
Sunday 20 September 2009             7.00am – 12.45pm

The Congress Managers will be onsite for enquiries at other times.

Insurance

Registration fees do not include insurance of any kind. It is strongly recommended that at the time you register for the Congress and book your travel you take out an insurance policy of your choice. The policy should include loss of fees/deposit through cancellation of your participation in the Congress, or through cancellation of the Congress, loss of international/domestic airfares through cancellation of any reason, loss of tour monies through cancellation for any reason including airline or related services strikes within Australia, failure to utilise tours or pre booked arrangements due to airline delay, Force Majeure or any other reason, medical expenses (including sickness and accident cover), loss or damage to personal property, additional expenses and repatriation should travel arrangements have to be altered. The Congress Managers cannot take any responsibility for any participant failing to arrange their own insurance. This insurance is to be purchased in your country of origin.

Privacy

Australia introduced the Privacy Amendment (Private Sector) Act 2000 in 2001. The Congress Managers comply with such legislation which is designed to protect the right of the individual to privacy of their information. Information collected in respect of proposed participation in any aspect of the Congress will be only used for the purposes of planning and conduct of the Congress and may also be provided to the organising body or organisers of future events. It is also usual to produce a ‘Delegate List’ of attendees at the Congress and to include the individual’s details in such a list. Consent for publication of the individual’s information may be withheld when completing the Registration Form for the Congress. Individuals are also entitled to access the information held by written application to the Congress Managers.

Private Touring/Hospitality Arrangements

Should you have any specialised group requirements for private touring, transportation or hospitality services other than those organised for the official program, please contact the Congress Managers, who would be please to assist with arrangements.

Private Functions

It is requested that you advise the Congress Managers of any private/corporate functions that might be planned outside the official program. The Congress Managers would be please to assist with arrangements for any of these events.
