Registration

Participation is open to all persons interested in Cardiology and Cardiovascular Health. Those who wish to attend the Meeting should complete the online registration form. Online registration is strongly encouraged.

Please note that all persons intending to attend the Meeting must register, including Invited Speakers, Chairpersons, Delegates, Accompanying Persons, Exhibitors and Sponsors. All Invited Speakers, Sponsors and Exhibitors will be contacted by Tour Hosts with an online registration form.
Please read through the following information carefully before you complete the online form.

Student Registrations
Student registrations include entry to all sessions, tea breaks, lunches, satchel and Meeting handbook. Delegates registering at the student rate must be full time students enrolled at a tertiary institution. Students must submit a letter from their supervisor defining their student status and include a copy of their current student card with the registration form when registering for the Meeting. This letter will need to be emailed to csanz2009@arinex.com.au.

1 Research Fellows / Advanced Trainees
2 Research Student Registration – this includes both higher degree and honours student
3 Medical Student
** Free registration is offered to the first 20 registrations received in the Medical Student category.

Payment of Fees

Credit Card Payment Online:
Online Registration is preferred. Payment for online registrations by credit card can be made by completing the online form and selecting your payment method as 'Online Credit Card Payment'. MasterCard, Visa, Diners Club and American Express are the only credit cards accepted at the Meeting. Please note all transactions by credit card will appear on your statement as payment to ‘Conference by THPL’.

Cheque/Money Order Payments:
All payments by cheque incur a $10 processing fee.

Payment by cheque can be made by completing the online registration form and selecting your payment method as 'Cheque Payment'. An acknowledgement will be emailed to you with a tax invoice attached.
Cheque payments will only be accepted up until 8 July 2009. After this date, all registrations (including accommodation bookings) must be submitted with credit card payment via the online registration form or by calling +61 2 9265 0700.
Cheques are to be made payable to ‘CSANZ 2009 ASM’. Your name and full address should be typed or printed clearly on the back of the cheque. Cheque payments from overseas must be a bank cheque in Australian Dollars drawn on an Australian Bank. No other currency will be accepted.

Faxing/Mailing Registrations:
Faxed or posted registration forms will only be  processed / confirmed if credit card details or cheque payment are accompanied with the form.
 
You may not pay your fees by Electronic Funds Transfer (EFT).

Please note: As this is a COMBO registration form for two separate Conferences, there will be two sections to fill in. You will be required to complete the CSANZ 2009 ASM registration form first, including any accommodation requirements for your entire stay, and secondly the CSANZ Indigenous Cardiovascular Health Conference 2009 registration form. Please ensure you complete the entire process.

Acknowledgements

Your registration and payment will be acknowledged via email or post, with confirmation of your requirements according to your registration form, and a copy of your tax invoice will be attached. Your registration will not be processed or confirmed if payment does not accompany your form. You will not receive any further publications before the Meeting. Your letter of acknowledgment will include any further advice necessary prior to your arrival at the Meeting. At the Meeting you will receive the final programme and the book of published papers/Meeting documentation. 

Cancellation And Refund Policy

Cancellations must be notified in writing to the Meeting Managers.

Registration Fees:

  • Before 18 May 2009 will incur an A$100.00 cancellation fee.
  • Before 8 July 2009 will receive a 50% refund of fees paid.
  • After 8 July 2009 there will be no refund.

Accommodation:  

  • A minimum one night’s deposit must be paid or credit card details given at time of booking to guarantee reservation. All deposits must be guaranteed by 8 July 2009.
  • Deposit is non-refundable at 8 July 2009

Social:

  • Payments are non-refundable at 8 July 2009

Entrance To Meeting

Each member of the Meeting will receive a name badge upon registration onsite. The badge will be your official pass and must be clearly visible at all times to obtain entry to all sessions and to social functions. The Meeting Managers expect appropriate behaviour from attendees and reserves the right to revoke the badge and privileges of anyone behaving in an inappropriate manner.

Insurance

Registration fees do not include insurance of any kind. It is strongly recommended that at the time you register for the Meeting and book your travel you take out an insurance policy of your choice. The policy should include loss of fees/deposit through cancellation of your participation in the Meeting, or through cancellation of the Meeting, loss of international/domestic air fares through cancellation for any reason, loss of tour monies through cancellation for any reason including airline or related services strikes within Australia, failure to utilise tours or pre booked arrangements due to airline delay, Force Majeure or any other reason, medical expenses (including sickness and accident cover), loss or damage to personal property, additional expenses and repatriation should travel arrangements have to be altered. The Meeting Managers cannot take any responsibility for any participant failing to arrange their own insurance. This insurance is to be purchased in your country of origin.
