Participation is open to all persons interested in Cancer Pain. Those who wish to attend the Symposium should read the detailed information and complete the Registration Form online. Each delegate must complete a separate form. If you require a hard copy form, please click here to download the form.
Please note that all persons intending to attend the Symposium must register, including Keynote Speakers.

NOTE: Registration will not be processed or confirmed until payment in full is received.
Within Australia
Payment may be made by personal cheque until the commencement of the Symposium or credit card. Bankcard, Mastercard and Visa are the only credit cards accepted at the Symposium. Please note all transactions by credit card will appear on your statement as payment to "Conference by THPL". Cheques should be made payable to " Cancer Pain 2005 Symposium".
International
Payments from overseas must be made by credit card or bank cheque in Australian Dollars only, drawn on an Australian Bank. Your name and full address should be typed or printed clearly on the back of the cheque. You may not pay your fees by direct transfer.
Acknowledgments
Your registration and payment will be acknowledged by email with confirmation of your requirements according to your registration form.
You will not receive any further publications before the Symposium. Your letter of acknowledgment will include any further advice necessary prior to your arrival at the Symposium.
Cancellation and refund policy

Certificate of insurance
A certificate of attendance will be issued upon request, at the conclusion of the symposium. Please ask at the Registration Desk.
Letter of invitation
On request, the Organising Committee will send a letter of invitation to participate in the Symposium. Such an invitation is extended specifically to assist participants to obtain travel funds or appropriate visas and does not imply any commitment on the part of the Organiser to provide financial support. The letter will be forwarded to you with confirmation of your registration.
Passport and Visa
All travellers to Australia , other than New Zealand , require a valid entry VISA prior to travel to Australia . For many nationalities (29 in total) an Electronic Visa or Electronic Travel Authority (ETA) can be obtained from your travel agent or airline when making your travel arrangements. Insurance
Registration fees do not include insurance of any kind. It is strongly recommended that at the time you register for the Symposium and book your travel you take out an insurance policy of your choice. The policy should include loss of fees/deposit through cancellation of your participation in the Symposium, or through cancellation of the Symposium, loss of international/domestic air fares through cancellation for any reason, loss of tour monies through cancellation for any reason including airline or related services strikes within Australia, failure to utilise tours or pre booked arrangements due to airline delay, Force Majeure or any other reason, medical expenses (including sickness and accident cover), loss or damage to personal property, additional expenses and repatriation should travel arrangements have to be altered. The Symposium managers cannot take any responsibility for any participant failing to arrange their own insurance. This insurance is to be purchased in your country of origin.
